what happened是一般过去时，强调动作发生在过去，但与现在无关。
what has happened ？是现在完成时，动作也发生在过去，但对现在还有影响或结果，也就是说结果仍然存在。不知你明白了吗？
这两句应该有上下文或有时间状语。
后者在地道的英语里一般不使用。“发生了什么”有固定句式，只用过去时：what happened ？
